The following documents with the compulsory RFQ Bid Form are to be submitted in the sealed envelope:

          1.    DRC Compliance Documents:

• RFQ Form - must be signed and stamp by authorized company representative.
• DRC Technical Bid Form for goods and Annex A.1. - completed, signed, and stamped by authorized company representative
• DRC Financial Bid Form for goods and Annex A.2. - completed, signed, and stamped by authorized company representative
• DRC Supplier Profile and Registration Form – must be completed, signed, and stamped by authorized company representative.
• DRC Supplier Code of Conduct - must be signed by authorized company representative.
• DRC General Conditions of Contract - must be signed by authorized company representative.
• Company’s Registration Certificate issued by the relevant state authority.
• Copy of ID majority owner.
• Annex A.3 - signed and stamped by authorized company representative.

2. Technical mandatory Documents:

• Time frame.
• List of machinery and equipment.
• Reference list for the previous works in the field with all related information that should include, but is not limited to: Beneficiary name, work description, contract value.
• Evidence on realized key contracts (copy of signed contract and certificate on the technical acceptance or final bill or reference letter(s) or similar document).
• List of key staff included in the works.
• Notice for participation in public tenders for works in the field of constructions and installations (from Ministry of Infrastructure) 
• Recommendation letters

3. Administrative mandatory Documents:

• Company’s registration documents - Registration document issued by the public body in the country of residence.
• Documents proof for responsible contractors (engineers): copy of diplomas and CVs.
• Confirmation of the Local Tax Authority office that all tax liabilities have been settled (not older than 30 days before the date of Tender publication)/ Certificate that demonstrates absence of debts to the state.
• Duly authorized signature (an official document - statutes, power of attorney, notary statement, etc.- proving that the person who signs on behalf of the company/joint venture/consortium is duly authorized to do so).
• Balance sheet and PROFIT/LOSS BALANCE (for at least the last three years for which accounts have been closed)/Financial report.

Performance Security is required in the format provided in this document for a value of not less than 5%. This should be valid for 30 days past the contract completion period including any defects liability period or any warrantee period. The period of Bid validity shall be 60 days after the deadline for Bid submission specified in the Bidding Data (from the winning supplier)

THE RFQ BID FORM CANNOT BE EMAILED TO ANY OTHER DRC EMAIL ADDRESS

The sealed envelope must be deposited into the DRC Tender Box at the address stated on page one before the RFQ Closing Date and Time. It is the Bidders responsibility to ensure that the sealed envelope is deposited into the Tender Box.

Any Bids not received on the official DRC Bid Form, or in a sealed envelope may be disqualified for non-compliance with these RFQ Instructions.
